Please note: CONSENT TO TRAVEL FORM

  1. All parents MUST COMPLETE the Consent to Travel form
  2. DO NOT SEND IT TO US
  3. Please PRINT THE FORM
  4. Your son or daughter MUST BRING IT WITH THEM
  5. They will need to SHOW IT to immigration at the airport
